Print Waiver

Waivers must be completed by all persons over the age of 18 participating in Guided Education Program. Minors (those under the age of 18) need to be claimed on only one parent/guardian’s waiver. Waivers are available at the Turtle Hospital, but it is highly recommended that you print and sign one ahead of time to speed up the check in process.
